- Click here to create an account
- Click Intake Forms at the top of the page, enter requested information, and sign electronically
- Click Billing & Insurance
- Fill out Bill To section
- Click Enter New Card if you wish to make payments via the portal
- If you have BlueCross BlueShield of Illinois, click Add Insurance and enter your insurance information
- Click your name in the upper right and then Demographic Info
- Fill out Address and Birth Date fields
